Help for elderly folk with chronic ailments  thumbs up

Applying for the Scheme.......What the benefits are ........

From next month ( January 2009 ) needy elderly can be subsidised at private GP clinics for treatment  of acute conditions such as colds and coughs, as well as chronic conditions such as High Blood Pressure, High Cholestrol and Diabetes.

Who can apply.

Singaporeans 65 years old and above earning less than $800 per capita monthly house-hold income can apply for the Community Medical Benefit Card.

How to apply.

Just walk into any Community Centre or Community Development Council to apply for the card with your particulars.

SERVICES FOR AMBULANT ELDERLY

SENIORS ACTIVITY CENTRES

Senior Activity Centres (SACs), located at selected one-room rental blocks, reach out to vulnerable elderly, especially the lower income who live alone. The centres organize a variety of activities to engage the elderly and provide information and referral services to those who need help.
 
LIONS BEFRIENDERS

The Lions Befrienders Service has a long history in helping the elderly cope with daily living and integrating them in the community. Volunteers visit and befriend lonely, frail elderly who live in 1- or 2-room HDB flats and have no family support or network

SENIOR CITIZENS CLUBS

The People's Association manages a network of 433 Senior Citizens Clubs (SCC) which has more than 200,000 members. They are located in community centres or residents' committees clubs and organizes activities to keep the well actively socially engaged in the community.

SUPPORT SERVICES FOR THE FRAIL ELDERLY

COMMUNITY CASE MANAGEMENT SERVICE

Community Case Management Service (CCMS) is a collaborative process that assesses, implements, co-ordinates, monitors and evaluates care plans and services for the elderly who has multiple and complex needs while being cared for in the community. The services aim to link the elderly and/or caregivers to different services without having to go to different providers.

HOME HELP SERVICE

The Home Help Service (HHS) is a community support service to assist elderly persons who are unable to look after themselves or maintain the normal functioning of their households. These elderly may not be chronically sick or frail but in the natural process of ageing, they might not be able to perform certain household functions.

DAY CARE CENTRE FOR SENIOR CITIZENS

The Day Care Centre (DCC) is a community-based facility; conveniently located at the void deck spaces in high elderly-populated neighbourhoods.
There are currently 18 DCCs under the purview of MCYS with average capacity of 50 places each for maintenance of their functional abilities.

At this recession and downturn, the authorities uses many channels to reach out to Singaporeans who needs help.

1. For major assistance schemes, wide publicity is done through the mass media. TV, radio and publications.

2. GST credits, household benefits are by notification letters to Singaporeans.

3. Households will soon receives an informational flier on benefits they can receive from the Resilence Package.

4. Roadshows and briefings are organised for grassroots, who in turn will help needy residents and elders to sign up for GST credits / senior citizen bonus.

5. Fliers containing general informations on WIS and WIS special payments are also available at the CC's.

Singaporeans who need more  information can also contact the Community Development Councils (CDC'S), Family Service Centres (FSC'S)  for assistance. Called 1800-222-0000 if in doubt. thumbs up
